As a key member of our clinical team, you will provide high-quality, patient-centered care to individuals in their homes, working closely with physicians and other healthcare professionals to develop and implement personalized care plans.
Responsibilities- Develop and implement comprehensive care plans in accordance with physician orders and patient needs.
- Provide education and support to patients and their families to promote optimal health outcomes.
- Collaborate with primary caregivers to ensure seamless care transitions and effective communication.
- Coordinate with healthcare teams to ensure timely and efficient care delivery.
- Perform a range of skilled nursing tasks, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
- Active RN license in the state of application.
- Valid CPR certification and driver's license.
- 1 year of RN experience in a healthcare setting (preferred).
