Segue Health Management Corp is on the lookout for a dedicated Patient Care Coordinator to enhance our team. This role is essential in managing post-discharge transitional care services. The ideal candidate will possess a combination of clinical knowledge, outstanding organizational skills, and effective communication abilities.

  • Required Experience: A background in the healthcare sector is essential.
  • Technical Skills: Proficiency in Microsoft Excel and familiarity with EMR/EHR systems are necessary.
  • Transportation: Must have access to transportation for local healthcare facility visits and the ability to work remotely when required.
  • Compassionate Approach: A genuine passion for assisting others is crucial.
  • Key Responsibilities: This role involves collaborating with referring healthcare providers to identify eligible Transitional Care Management (TCM) patients, explaining the TCM program to referrals, maintaining accurate referral and discharge logs, updating electronic medical records, conducting follow-up calls with patients post-discharge, and organizing home visits.
  • Preferred Background: Candidates with experience in social work, nursing, or home health are encouraged to apply. Attention to detail and strong organizational capabilities are vital.

Employment Type: Full-time

Compensation: Salary range of $35,000 to $45,000 annually
