Medication Management Technician

A Medication Management Technician plays a vital role in assisting healthcare teams to ensure patients receive the right medication therapy. This position involves close collaboration with pharmacists, nurses, physicians, and other healthcare professionals to effectively manage patients' medication requirements.
Key Responsibilities:- Medication Review:
- Assist in evaluating patients' medication histories to ensure accurate documentation.
- Identify and rectify discrepancies in medication records in collaboration with pharmacists.
- Patient Guidance:
- Educate patients on their medications, including usage instructions, potential side effects, and the significance of adherence.
- Address patient inquiries or refer them to appropriate healthcare providers for complex questions.
- Adherence Monitoring:
- Monitor and track patients' adherence to medication regimens using electronic health records (EHR) and other tools.
- Follow up with patients who miss doses or are at risk of non-adherence, reporting any concerns to the care team.
- Care Coordination:
- Facilitate communication among healthcare providers to ensure smooth transitions of care, especially during patient discharges or changes in care settings.
- Assist in coordinating medication refills, prior authorizations, and delivery services.
- Data Management:
- Accurately input and update patient information in the EHR system.
- Document all patient interactions, interventions, and communications with healthcare providers.
- Support for Healthcare Teams:
- Assist pharmacists in conducting medication evaluations and identifying potential drug interactions.
- Provide data on medication usage patterns to support the care management team in developing care plans.
- Regulatory Compliance:
- Ensure all activities adhere to relevant regulations, including HIPAA and state pharmacy laws.
- Participate in quality improvement initiatives aimed at enhancing patient care and safety.
- Certification: Must hold current certification as a Pharmacy Technician (CPhT) from a recognized certifying body.
- Experience: Previous experience in a pharmacy setting, particularly in a care management context, is preferred.
- Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and chronic disease management.
- Technical Skills: Proficient in using electronic health records (EHR) systems, pharmacy software, and telehealth tools.
- Communication: Excellent verbal and written communication skills for effective interaction with patients and healthcare providers.
- Attention to Detail: High accuracy in documenting patient information and processing medication orders.
- Problem-Solving: Ability to identify medication-related issues and collaborate on solutions.
