Job Overview

A Medication Management Technician plays a vital role in assisting healthcare teams by ensuring that patients receive optimal medication therapy. This position involves collaboration with pharmacists, nurses, physicians, and other healthcare professionals to address patients' medication requirements.

Key Responsibilities:
  1. Medication Review:
    • Assist in evaluating patients' medication histories to ensure accurate documentation.
    • Identify and rectify discrepancies in medication lists in partnership with pharmacists.
  2. Patient Guidance:
    • Educate patients on their medications, including administration, side effects, and adherence importance.
    • Respond to patient inquiries or refer them to the appropriate healthcare professional for complex questions.
  3. Adherence Monitoring:
    • Utilize electronic health records (EHR) to track and monitor patients' medication adherence.
    • Follow up with patients who miss doses or are at risk of non-adherence, reporting any concerns to the care team.
  4. Care Coordination:
    • Facilitate communication among healthcare providers to ensure smooth transitions of care, especially during hospital discharges or changes in care settings.
    • Assist in coordinating medication refills, prior authorizations, and delivery services.
  5. Data Management:
    • Accurately input and update patient information within the EHR system.
    • Document all patient interactions, interventions, and communications with healthcare providers.
  6. Support for Healthcare Teams:
    • Assist pharmacists in conducting medication evaluations and identifying potential drug interactions.
    • Provide the care management team with data on medication usage patterns to aid in developing care plans.
  7. Regulatory Compliance:
    • Ensure all activities adhere to relevant regulations, including HIPAA and state pharmacy laws.
    • Participate in quality improvement initiatives aimed at enhancing patient care and safety.
Qualifications:
  • Certification: Current certification as a Pharmacy Technician (CPhT) from a recognized certifying body.
  • Experience: Previous experience in a pharmacy environment, preferably within care management.
  • Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and chronic disease management.
  • Technical Skills: Familiarity with electronic health records (EHR) systems and pharmacy software.
  • Communication: Excellent verbal and written communication skills for effective interaction with patients and healthcare providers.
  • Attention to Detail: High accuracy in documenting patient information and processing medication orders.
  • Problem-Solving: Ability to identify medication-related issues and collaborate to resolve them.
