Position Summary

The Adult Homes Initiative aims to provide peer support services to individuals transitioning from adult homes into the community through both individual and group peer support sessions. Employees will leverage their lived experiences to assist individuals in moving from Adult Homes into their desired communities.

Key Responsibilities

The essential functions include, but are not limited to:

  • Fostering positive, supportive peer relationships that cultivate trust and hope for change, choice, wellness, recovery, and personal empowerment.
  • Serving as a positive role model for individuals with lived experiences who have successfully managed their recovery by actively maintaining their personal wellness.
  • Providing one-on-one and/or group support along with ongoing short-term support services following relocation.
  • Teaching community adjustment and wellness self-management skills.
  • Assisting in the development of a Wellness Recovery Action Plan (WRAP).
  • Helping individuals reconnect with family, friends, and other natural supports.
  • Offering education and resources that promote wellness self-management, self-determination, and self-advocacy.
  • Continuing support for individuals post-transition to identify and engage with various community-based services and supports.
  • Maintaining accurate and timely documentation of activities.
  • Regularly reporting to and meeting with supervisors.
  • Participating in required training and meetings as assigned.
  • Demonstrating a strong understanding and practice of peer support, recovery, wellness, and related services.
  • Exhibiting strong engagement, oral, and written communication skills.
  • Being available to travel and work during afternoons, evenings, and weekends.
  • Covering different homes in the designated borough as needed.
  • Performing additional duties as assigned.

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • High School Diploma or GED required; a college degree in a related field is preferred.
  • A minimum of one year of experience working as a peer.
  • New York State Certified Peer Specialists or must obtain certification within six months of hiring.
  • Experience and knowledge of the New York State mental health and substance abuse systems.
  • Familiarity with advocacy, self-help, and empowerment programs for recipients of mental health and substance abuse services.
  • Ability to serve as a role model and convey a message of hope and wellness for individuals diagnosed with mental illness.
  • Capability to fulfill basic documentation requirements regarding work activities.
  • Must possess lived experience with the public mental health system.
  • Basic computer skills are required; knowledge of Foothold Technology's AWARDS system is a plus.

COVID-19 Vaccination Requirement:

As a state-mandated organization, COVID-19 vaccinations are required. Selected candidates must provide proof of complete vaccination against COVID-19 and having received a booster shot prior to job offer extension. Exemptions may be requested for qualifying medical conditions or religious beliefs.

The noise level in the work environment is typically moderate.
