Job Summary

The Salvation Army is seeking a highly skilled Case Management Specialist to join our team. As a Case Management Specialist, you will be responsible for providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in our established life management program.

Key Responsibilities

  • Interviews, accepts, and provides case management services for clients
  • Develops comprehensive program plans and goals based on client needs
  • Evaluates client progress and conducts mentoring and counseling sessions
  • Serves as an advocate for clients to acquire necessary services
  • Ensures compliance with funding requirements

Requirements

  • Bachelor's degree in Social Work, Behavioral Science, or a related field
  • Three years of progressively responsible experience in case management social services
  • Approved background check and valid State Driver's License

Physical Requirements and Working Conditions

The Case Management Specialist will be required to:

  • Meet attendance requirements
  • Read, write, and communicate in English
  • Communicate clearly and effectively both orally and in writing
  • Think clearly and quickly to maintain control of client caseload
  • Work on multiple cases effectively and efficiently
