Case Manager

Job Summary
The Salvation Army is seeking a highly skilled Case Management Specialist to join our team. As a Case Management Specialist, you will be responsible for providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in our established life management program.
Key Responsibilities
- Interviews, accepts, and provides case management services for clients
- Develops comprehensive program plans and goals based on client needs
- Evaluates client progress and conducts mentoring and counseling sessions
- Serves as an advocate for clients to acquire necessary services
- Ensures compliance with funding requirements
Requirements
- Bachelor's degree in Social Work, Behavioral Science, or a related field
- Three years of progressively responsible experience in case management social services
- Approved background check and valid State Driver's License
Physical Requirements and Working Conditions
The Case Management Specialist will be required to:
- Meet attendance requirements
- Read, write, and communicate in English
- Communicate clearly and effectively both orally and in writing
- Think clearly and quickly to maintain control of client caseload
- Work on multiple cases effectively and efficiently
