Empowering Clients through Comprehensive Support
At Acuity International, we strive to provide exceptional support to our clients through our dedicated team of Case Managers. As a key member of our team, you will play a vital role in empowering our clients by actively leveraging our broad range of services.
Key Responsibilities:
- Coordinate case management services for individuals and families, ensuring a safe and clean environment.
- Identify potential family members in need of services and support them in understanding the process to obtain assistance.
- Develop and implement Individual Disaster Case Management Plans in accordance with State of FL policies and procedures.
- Collaborate with community resources and care coordinators to maintain effective communication and ensure good outcomes.
- Maintain accurate records and develop reports on case progress.
Requirements:
- Associate's degree in social work, psychology, sociology, or a related field.
- Clean criminal background check.
- Excellent communication and interpersonal skills, with the ability to multi-task and be detail-oriented.
- Proficiency in MS Office products and common office equipment.
