Job SummaryAs a Case Manager at AppleOne, you will play a vital role in creating a safe and stable environment for participants. You will utilize evidence-based best practices to provide mental health-focused, trauma-informed support, partnering with families and individuals to achieve their goals. This position requires a high degree of professionalism, commitment, and adaptability to uphold community values and maintain a collaborative environment.

Key Responsibilities Develop and implement individualized plans to support participants' needs
Provide mental health-focused, trauma-informed support and resources
Collaborate with participants, families, and community partners to achieve goals
Maintain accurate records and data management
Participate in bi-weekly meetings and case conferences

Requirements 2+ years of case management experience (or equivalent)
Bachelor's degree or certification in mental health area (Psychology, Social Work, Counseling, etc.)
Strong verbal and written communication skills
Ability to work with diverse populations and exercise good judgment
