Job Overview

AppleOne is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in creating a safe and stable environment for participants, utilizing evidence-based best practices and mental health-focused approaches.

Key Responsibilities
  • Develop and implement individualized service plans to meet the unique needs of participants
  • Provide case management services, including assessment, planning, and coordination of services
  • Collaborate with community resources and partners to ensure comprehensive support
  • Utilize trauma-informed and culturally sensitive approaches to engage participants and families
  • Maintain accurate and timely records of participant interactions and service delivery
Requirements
  • Minimum of 2 years of case management experience, preferably in mental health
  • Bachelor's degree in a related field, such as psychology, social work, or counseling
  • Strong communication and interpersonal skills, with ability to work with diverse populations
  • Ability to work independently and as part of a team, with minimal supervision
  • Proficiency in data management and problem-solving skills
