ROLE AND QUALIFICATIONS:
In the capacity of a Case Manager, you will be tasked with creating and overseeing service plans for families within housing programs, facilitating access to resources, and nurturing relationships. To excel in this position, a Bachelor's degree in social services, knowledge of crisis intervention, and strong communication skills are essential.
KEY RESPONSIBILITIES:
- Familiarity with community resources available for families experiencing homelessness.
- Develop housing solutions for program participants.
- Establish collaborative partnerships with governmental agencies.
- Assist families with housing applications and opportunities.
- Maintain accurate records and reports pertaining to housing objectives.
- Engage in quality improvement activities and life skills training.
- Perform additional assigned tasks as necessary.
