Job Summary

We are seeking a skilled and compassionate Social Worker to join our team at The Mary Wade Home Incorporated. As a Nursing Home Social Worker, you will play a vital role in ensuring the well-being and quality of life for our residents.

Key Responsibilities
  • Collaborate with residents, families, and healthcare providers to facilitate a smooth transition to our skilled nursing facility.
  • Develop and implement individualized service plans to maximize independence and promote resident well-being.
  • Provide guidance and support to residents and their families to navigate the healthcare system.
  • Utilize community resources to assist residents in resuming their daily lives.
  • Prepare and maintain accurate records of resident care and services.
Requirements
  • Bachelor's degree in Social Work or related field.
  • Minimum 1 year of Social Work experience, with preference given to those with skilled nursing experience and MDS expertise.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Flexibility to work a variety of shifts, including evenings and weekends.
