Responsibilities- Utilization Management:
- Monitor and address length of stay issues and level of care changes for compliance
- Document the case management plan, including clinical needs, barriers to quality care, and effective utilization of resources
- Collect and analyze data to identify patterns of care and potential avoidable delays in transition
- Care Coordination/Discharge Planning:
- Assess the individual's health status and identify needs for multi-disciplinary services
- Coordinate an interdisciplinary plan of care in partnership with the individual and their support services
- Develop and implement an effective discharge plan for complex-care patients
- Revenue Cycle:
- Demonstrate a working knowledge of financial and reimbursement processes to facilitate medical cost management
- Provide education to patients, families, staff, and physicians regarding financial aspects of care
- Facilitate empowerment of patients and families in self-management and healthcare decision-making
- Education:
- Bachelor of Art/Science Degree in a healthcare/human services related field or RN
- Master's degree in a healthcare/human services related field is highly encouraged
- Experience:
- Two years of clinical experience in focused areas working with multidisciplinary teams
- Licensure appropriate to population served
- Knowledge/Skills/Abilities:
- Writes, reads, comprehends, and speaks fluent English
- Basic computer knowledge using word processing, spreadsheet, email, and web browser
