Responsibilities
  • Utilization Management:
    • Monitor and address length of stay issues and level of care changes for compliance
    • Document the case management plan, including clinical needs, barriers to quality care, and effective utilization of resources
    • Collect and analyze data to identify patterns of care and potential avoidable delays in transition
  • Care Coordination/Discharge Planning:
    • Assess the individual's health status and identify needs for multi-disciplinary services
    • Coordinate an interdisciplinary plan of care in partnership with the individual and their support services
    • Develop and implement an effective discharge plan for complex-care patients
  • Revenue Cycle:
    • Demonstrate a working knowledge of financial and reimbursement processes to facilitate medical cost management
    • Provide education to patients, families, staff, and physicians regarding financial aspects of care
    • Facilitate empowerment of patients and families in self-management and healthcare decision-making
Qualifications
  • Education:
    • Bachelor of Art/Science Degree in a healthcare/human services related field or RN
    • Master's degree in a healthcare/human services related field is highly encouraged
  • Experience:
    • Two years of clinical experience in focused areas working with multidisciplinary teams
    • Licensure appropriate to population served
  • Knowledge/Skills/Abilities:
    • Writes, reads, comprehends, and speaks fluent English
    • Basic computer knowledge using word processing, spreadsheet, email, and web browser
