Care Manager

CVS Health is seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in improving the health outcomes of our members by assessing their needs, developing individualized care plans, and coordinating with healthcare providers to ensure seamless care transitions.
This is a challenging and rewarding role that requires strong communication and organizational skills, as well as the ability to work independently and as part of a collaborative team.
Key Responsibilities- Assess member needs and develop individualized care plans to address gaps in care and improve health outcomes
- Coordinate with healthcare providers to ensure seamless care transitions and optimize member care
- Communicate effectively with members, families, and healthcare providers to ensure that member needs are met
- Participate in interdisciplinary team meetings to discuss member care plans and share best practices
- Identify and link members with community resources and benefits to support their health and well-being
- 3+ years of experience in a healthcare-related field
- 2+ years of customer service experience
- 2+ years of experience with claims and healthcare data
- Strong organizational and communication skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office and electronic health records
- Master's degree in a healthcare-related field
- CRC, CDMS, CRRN, COHN, or CCM certification
- Medicare and Medicaid experience
- Managed care experience
- Experience working with geriatric special needs, behavioral health, and disabled populations
