Nurse Care Manager

We are seeking a skilled Nurse Care Manager to join our team at Intermountain Health. As a key member of our healthcare team, you will work collaboratively with physicians, interdisciplinary teams, and patients to promote positive patient outcomes.
Key Responsibilities- Assess, plan, implement, coordinate, monitor, and evaluate patient care needs using the nursing process.
- Provide focused support to various areas, including utilization management, emergency department, acute, ambulatory, and specialty care.
- Establish collaborative partnerships with patients to assist them in examining patterns of healthcare needs, decisions, lifestyle choices, and utilization of resources that affect their health.
- Advocate, educate, and coach patients, families, and caregivers about treatment options, community resources, and psychosocial concerns to set goals and help patients develop self-care skills and independence.
- Facilitate communication and coordination between members of the healthcare delivery team, involving patients in the decision-making process to minimize fragmentation in services.
- Graduate of an accredited Registered/Professional Nursing program.
- State licensure as a Registered Nurse (RN).
- Preferred qualifications include a Bachelor's degree in Nursing, Certified Case Manager (CCM), and relevant certification.
- 2 years' nursing experience in outpatient care and 1 year of Care Management experience.
- Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
