Pharmacist - Anticoagulation Specialist

Confluence Health is seeking a highly skilled Pharmacist to join our team as an Anticoagulation Specialist. As a key member of our interdisciplinary care team, you will be responsible for providing clinical and operational pharmacy services to patients in our ambulatory setting.
Key Responsibilities- Manage medication therapy and order appropriate labs based on published medical literature, Confluence Health guidelines, evidence-based protocols, and individual patient clinical parameters.
- Enter progress notes with additional information about medication therapy management decisions.
- Ensure all orders placed by the pharmacist are pursuant to an established protocol initiated by the provider or co-signed by an authorized provider to meet conditions of payment.
- Maintain competence in CPT coding and pass regular compliance audits.
- Communicate with patients, family members, and other healthcare providers to promote coordinated care.
- Advocate for and support formulary management and pharmacy and therapeutics initiatives.
- Participate in department and organizational performance initiatives.
- Provide direct supervision and oversight to clinical staff members.
- Commit to instructing, modeling, and coaching staff, students, and residents.
- Report adverse drug events and mitigate future patient safety risks through participation in medication quality activities.
- Maintain active credentialed status via Confluence Health delegated credentialing through the Medical Staffing Office.
- Required to maintain active Basic Life Support (CPR) certification.
- Required: Bachelor of Science in Pharmacy (RPh) and licensed to practice in the state of Washington prior to start date.
- Desired: Pharm. D, bilingual with ability to pass a bilingual fluency exam with a CH chosen vendor, and three years of experience as a clinical pharmacist in the ambulatory setting or successful completion of a pharmacy practice residency with ambulatory care experience.
This position requires frequent walking, sitting, and standing, as well as occasional reaching, climbing, and lifting. The employee may be exposed to infectious patients or materials, hazardous and toxic substances, and musculoskeletal injuries if proper lifting and carrying techniques are not used.
