Job Summary

We are seeking a highly skilled and experienced ARNP to join our team at Confluence Health. As an Anticoagulation Specialist, you will play a critical role in the clinical management of patients on anticoagulation therapy.

Key Responsibilities
  • Monitor medication therapy and perform ongoing assessments of patient risk factors, indications for therapy, and work towards optimization of therapeutic regimens.
  • Interpret medical records and test results, and adjust medication therapy in accordance with Confluence Health-approved guidelines, evidence-based practice, and department standards.
  • Maintain competence in CPT coding and pass regular compliance audits.
  • Document patient care activities, patient and family teaching, and communication with the patient care team in the electronic medical record.
  • Communicate with patients, family members, and other healthcare providers to promote coordinated healthcare across the continuum.
  • Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Participate in department-driven practices, including provider huddles, journal clubs, and on-call scheduling.
Requirements
  • Active Registered Nurse and ARNP licensure with prescriptive authority in the state of Washington, required by start date.
  • Related experience, 3 years preferred.
  • The following certification is preferred and will be required within 24 months of exam eligibility: CACP.
Working Conditions

The work is performed at in-patient exam and treatment rooms, where space may be limited and confined. Requires a combination of working independently and also working closely with others.
