We are seeking a highly skilled and experienced ARNP to join our team at Confluence Health. As an Anticoagulation Specialist, you will play a critical role in the clinical management of patients on anticoagulation therapy.
Key Responsibilities- Monitor medication therapy and adjust dosages as needed to optimize patient outcomes.
- Develop and coordinate perioperative and procedural plans to ensure safe and effective anticoagulation management.
- Counsel patients and family members on anticoagulation-related issues and provide education on disease states and medication therapy.
- Coordinate communication with healthcare providers to ensure seamless care coordination.
- Act as a resource to the integrated care team on anticoagulation medication management.
- Active Registered Nurse and ARNP licensure with prescriptive authority in the state of Washington.
- Related experience, preferably 3 years.
- Preferred certification: CACP.
This role requires working in a fast-paced clinical environment, with a focus on patient safety and quality care. The successful candidate will be able to work independently and collaboratively as part of a team.
