Job Summary

We are seeking a highly skilled and experienced ARNP to join our team at Confluence Health. As an Anticoagulation Specialist, you will play a critical role in the clinical management of patients on anticoagulation therapy.

Key Responsibilities
  • Monitor medication therapy and adjust dosages as needed to optimize patient outcomes.
  • Develop and coordinate perioperative and procedural plans to ensure safe and effective anticoagulation management.
  • Counsel patients and family members on anticoagulation-related issues and provide education on disease states and medication therapy.
  • Coordinate communication with healthcare providers to ensure seamless care coordination.
  • Act as a resource to the integrated care team on anticoagulation medication management.
Requirements
  • Active Registered Nurse and ARNP licensure with prescriptive authority in the state of Washington.
  • Related experience, preferably 3 years.
  • Preferred certification: CACP.
Working Conditions

This role requires working in a fast-paced clinical environment, with a focus on patient safety and quality care. The successful candidate will be able to work independently and collaboratively as part of a team.
