Summary:

The Clinical Pharmacist is tasked with delivering both clinical and operational pharmacy services, adhering to the highest standards of pharmaceutical care for each patient within their designated area.

This role encompasses therapeutic evaluations, interventions, and care plans tailored for individual patients as well as broader patient populations. The pharmacist plays a crucial role within the Interdisciplinary Care Team, taking responsibility for medication-related outcomes and aligning with the pharmacy enterprise's vision to enhance patient outcomes.


Position Reports To:
Pharmacy Manager

Essential Functions:

The pharmacist will oversee medication therapy and order relevant laboratory tests based on established medical literature, Confluence Health guidelines, evidence-based protocols, and the specific clinical parameters of the patient.

They will document accompanying progress notes to provide additional context regarding medication therapy management decisions.

All orders placed by the pharmacist will comply with established protocols initiated by the provider or will be co-signed by an authorized provider to ensure payment conditions are met.

Maintaining proficiency in CPT coding and passing regular compliance audits is essential.

Effective communication with patients, family members, and other healthcare professionals is vital, serving as a liaison to promote coordinated healthcare across the continuum.

The pharmacist will advocate for formulary management and participate in pharmacy and therapeutics initiatives, contributing to departmental and organizational performance goals.

Direct supervision and oversight of clinical staff members is required, along with a commitment to instructing, modeling, and coaching staff, students, and residents.

Reporting adverse drug events and mitigating future patient safety risks through involvement in medication quality activities is expected.

Demonstrating competency and proficiency in essential job functions is required by maintaining active credentialed status through Confluence Health's delegated credentialing.

Active Basic Life Support (CPR) certification is mandatory.

Other duties may be assigned as necessary.

Position Requirements:

Full vaccination against COVID-19 is a prerequisite for employment at Confluence Health, with religious and medical exemptions considered upon request.


Qualifications:

Required:
Bachelor of Science in Pharmacy (RPh).
Must be licensed to practice in the state of Washington prior to starting.

Desired:
Pharm. D
Bilingual candidates who can pass a bilingual fluency exam with a designated vendor are preferred.

Three years of experience as a clinical pharmacist in an ambulatory setting or successful completion of a pharmacy practice residency with ambulatory care experience is required.
