Position Summary:
Please Note: This is a pool position with no guaranteed hours.

The Clinical Pharmacist is tasked with delivering exceptional clinical and operational pharmacy services, adhering to the highest standards of pharmaceutical care for each patient. Responsibilities include conducting therapeutic assessments, implementing interventions, and developing care plans tailored to individual patients and broader patient populations. The pharmacist plays a crucial role within the Interdisciplinary Care Team, ensuring optimal medication-related outcomes and aligning with the pharmacy enterprise's vision of enhancing patient value.

Reporting Structure:
This position reports to the Pharmacy Manager.

Key Responsibilities:
1. Manage medication therapy and order necessary laboratory tests based on established medical guidelines and individual patient parameters.

2. Document progress notes that provide insight into medication therapy management decisions.

3. Ensure all pharmacist-initiated orders comply with established protocols and are co-signed by authorized providers when necessary.

4. Maintain proficiency in CPT coding and successfully pass regular compliance audits.

5. Communicate effectively with patients, families, and healthcare providers to promote coordinated care.

6. Advocate for formulary management and participate in pharmacy and therapeutics initiatives.

7. Contribute to departmental and organizational performance improvement efforts.

8. Supervise clinical staff and provide guidance and training to students and residents.

9. Report adverse drug events and engage in quality improvement activities to enhance patient safety.

10. Maintain active credentialed status through Confluence Health's delegated credentialing process.

11. Hold current Basic Life Support (CPR) certification.

12. Perform other duties as assigned.

Qualifications:
Required:

  • Bachelor of Science in Pharmacy (RPh).
  • Active license to practice in the state of Washington prior to commencement.

Desired:

  • Pharm. D
  • Bilingual with the ability to pass a bilingual fluency exam.
  • Three years of experience as a clinical pharmacist in an ambulatory setting or completion of a pharmacy practice residency with ambulatory care experience.
