The CHS Physician Partners Medical Group is a vital component of Catholic Health Services, employing approximately 450 healthcare professionals across over 90 community-based locations.


This medical group encompasses a diverse range of specialties, with many practitioners recognized as leaders in their fields.

Position Summary:

The Clinical Care Specialist (Advanced Practice Provider) plays a crucial role in the healthcare delivery system, providing primary care across various outpatient settings within the Catholic Health Physician Enterprise.


Working under the guidance of a licensed physician, the Clinical Care Specialist is responsible for delivering patient care and is credentialed through the Medical Staff process.

Key Responsibilities:

  • Conducting thorough medical histories and comprehensive physical examinations.
  • Diagnosing medical conditions and determining appropriate treatment plans.
  • Ordering and interpreting diagnostic tests.
  • Prescribing medications and performing necessary procedures.
  • Providing patient education and counseling as appropriate.
Workload:

Approximately 60% administrative duties and 40% direct patient care.

Qualifications:

  • For Physician Assistants: Completion of an accredited Physician Assistant program and a minimum of a Bachelor's degree.
  • For Nurse Practitioners: Current RN license and certification from AANP or ANCC.
  • Certification as a Physician Assistant (PA-C) or Nurse Practitioner (ACNP).
  • Current BLS and ACLS certifications.
Performance Responsibilities:

Collaborates effectively with physicians to ensure high-quality patient care.

Adheres to established protocols for treatments and procedures.

Welcomes patients and verifies their identity using two identifiers.

Completes detailed assessments of patients, including vital signs and medical history.

Provides telephone triage and facilitates appointment scheduling.

Evaluates patient responses and adjusts care plans as necessary.

Prescribes medications and provides ongoing patient education.

Reviews laboratory results and addresses critical findings promptly.

Contributes to quality performance reports and patient engagement initiatives.

Maintains a safe and efficient clinical environment, ensuring compliance with infection control guidelines.

Participates in team meetings to enhance communication and collaboration.

Assists in training new staff and may mentor student clinicians.

Strives to improve patient satisfaction and addresses any concerns raised by patients.

Ensures proper operation and maintenance of clinical equipment.

Adheres to protocols for the disposal of biohazardous materials.
