Assistant Health Services Director

The Assistant Health Services Director plays a vital role in ensuring the highest quality of care for residents at Prestige Senior Living - Southern Hills. Reporting directly to the Health Services Director, this LPN/LVN will assist in overseeing health services procedures and systems, care staff, and schedules. This position requires a strong commitment to resident-centered care, with a focus on dignity, respect, and individualized service plans.
Key Responsibilities:
- Partner with health services leadership to create a resident-centered approach to care, supporting residents' rights to participate in decision-making and treating residents with dignity and respect.
- Assist with assessments and evaluations of residents' health, functional, and psychosocial status, and participate in the development of individualized service plans.
- Evaluate residents with reported changes in condition, provide team member direction, and coordinate care to meet residents' changing needs.
- Collaborate with the management team on the move-in of prospective residents, considering their needs, community acuity, and staffing levels.
- Coordinate resident health services appointments, including transportation and use of the company vehicle.
- Communicate regularly with residents' families regarding health care needs, provide education and support, and assist in supervision and training of health services team members.
- Participate in the investigation and root cause analysis of incidents, collaborate with the management team on interventions, and ensure the documentation and communication of resident care and services.
- Assist in the management of Infection Control, provide training to staff, investigate and report incidents, and track infection data.
- Provide on-call support and phone triage for team members' questions regarding resident concerns, and work as a Medication Aide and/or Personal Care Attendant as needed.
Requirements:
- Current, unencumbered LPN/LVN license.
- Excellent written and oral communication, computer, and motivational skills.
- Ability to creatively problem-solve in resident care and employee management situations.
- Experience in geriatric nursing, medication administration, case management, training/staff development, budgets, and team management a plus.
