Job Summary

We are seeking a highly skilled and compassionate LPN to join our team as an Assistant Health Services Director at Prestige Senior Living - Southern Hills. As a key member of our health services leadership team, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Collaborate with health services leadership to develop and implement resident-centered care plans that meet the unique needs of each individual.
  • Assist with assessments and evaluations to identify residents' health, functional, and psychosocial needs, and develop individualized service plans to address these needs.
  • Provide leadership and guidance to care staff, including training and development opportunities to ensure they have the skills and knowledge needed to provide exceptional care.
  • Work closely with the management team to ensure the smooth operation of health services, including scheduling, staffing, and budgeting.
  • Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions and address any concerns or issues that may arise.
  • Participate in quality improvement initiatives to ensure our community meets the highest standards of care and service.
Requirements
  • Current, unencumbered LPN/LVN license.
  • Excellent written and oral communication, computer, and motivational skills.
  • Ability to creatively problem-solve in both resident care and employee management situations.
  • Experience in geriatric nursing, medication administration, case management, training/staff development, budgets, and team management a plus.
