Primary Care Physician

As a Family Practice Physician at Universal Community Health Center, you will be integral in delivering thorough and empathetic healthcare services to a varied community, with a particular emphasis on underserved groups. The ideal candidate will embody our dedication to providing preventive care, managing chronic conditions, and addressing immediate medical needs for patients across all age groups.
Your role will involve a strong commitment to health education and disease prevention, collaborating with multidisciplinary teams and community partners to ensure comprehensive patient care. Key Responsibilities:
- Deliver primary healthcare services to patients of all ages, emphasizing preventive care, chronic disease management, and urgent care needs.
- Ensure patient care adheres to professional standards and ethical guidelines, guaranteeing that all patients receive evaluation and treatment without discrimination.
- Conduct comprehensive medical assessments, including physical examinations, medical history evaluations, and diagnostic processes.
- Formulate and execute detailed treatment plans based on clinical evaluations and evidence-based practices.
- Oversee chronic conditions such as diabetes, hypertension, and asthma through consistent monitoring, medication management, and patient education.
- Instruct patients on health maintenance, disease prevention, and lifestyle changes to enhance overall health outcomes.
- Coordinate care with specialists and other healthcare professionals as necessary, ensuring seamless and comprehensive patient management.
- Utilize medical licensure to manage and create resource schedules, optimizing the distribution of healthcare professionals and facilities to meet patient care requirements effectively.
- Maintain precise and timely documentation of patient interactions and treatment strategies in our electronic health records system.
- Engage in community health initiatives, health fairs, and educational programs to foster health and wellness within the community.
- Participate in quality improvement efforts at Universal Community Health Center, engaging in peer reviews, clinical audits, and compliance with regulatory standards.
Qualifications:
- Medical degree (MD or DO) from an accredited institution.
- Board certification in Family Medicine or Internal Medicine-Pediatrics.
- Valid state medical license and DEA registration.
- Ability to uphold patient confidentiality and comply with HIPAA regulations.
- Completion of residency training in Family Medicine or Internal Medicine-Pediatrics.
- Dedication to providing culturally competent care to a diverse patient demographic.
- Strong communication abilities and capacity to collaborate effectively within a multidisciplinary team.
- Experience or interest in serving underserved populations and familiarity with FQHC operations is preferred.
