Role Summary:
The Family Practice Physician will deliver ongoing, comprehensive health and wellness services, including preventive and medical care to families of all ages and backgrounds. This role ensures that the practice team plays a pivotal role in coordinating and managing patient care, providing optimal treatment for acute and chronic conditions, as well as preventive care across various health settings.
Key Responsibilities:
- Develop and manage a comprehensive health program for patients, incorporating preventive medicine, behavioral health, and community health initiatives.
- Assist patients with chronic conditions in navigating and accessing necessary healthcare services, including education and social support.
- Conduct thorough medical history assessments and gather data through patient interviews.
- Perform patient examinations, create diagnostic plans, and order necessary tests. Analyze findings and implement treatment strategies.
- Prescribe medications and evaluate their effectiveness based on patient conditions.
- Identify the need for specialist consultations and facilitate care as directed by other healthcare professionals.
- Engage in care procedures in accordance with training and demonstrated competencies.
- Provide both supportive and definitive care for primary care-related medical issues, based on established proficiency.
- Participate actively in departmental meetings and team huddles.
- Assess and advise on the medical supplies and equipment required for patient care.
- Manage care for high-risk patients to minimize hospital visits and enhance health outcomes.
- Maintain a registry for high-risk patients, documenting interventions and care plans.
- Ensure timely communication with the primary care provider and practice team to optimize patient management.
- Ensure that all patient communications are culturally and linguistically appropriate.
- Conduct patient interviews, update medical records, and document information in the electronic health record (EHR) system.
- Provide medication information for new prescriptions in accordance with established standards.
- Coordinate medical care with both internal and external specialists as necessary.
- Supervise and train clinical professionals and students in various health-related topics, including first aid and healthcare maintenance.
Qualifications:
- Bilingual (English/Spanish) preferred.
- Graduate of an accredited medical school with completed internship and residency as required by California licensure.
- Valid and current California medical license.
- Board Certification required.
- Current DEA registration.
- Preferred three years of experience in a medical practice.
- Strong interpersonal, organizational, verbal, and written communication skills; ability to thrive in a fast-paced, team-oriented environment.
- Ability to relate effectively to individuals from diverse socio-economic backgrounds.
- Self-motivated, adaptable, and a collaborative team player.
- Availability for evening and weekend clinics.
- Ability to work cooperatively as part of a healthcare delivery team.
