Role Summary:

The Hospice Certified Nursing Assistant (CNA) plays a vital role in the interdisciplinary team, working under the guidance of a registered nurse to provide essential personal care that addresses the needs of patients and enhances their comfort. Key responsibilities include adhering to care plans, monitoring and reporting on patient conditions, and maintaining accurate documentation of care provided. The primary goal of the CNA is to ensure high-quality, consistent, and safe care for patients.

Essential Qualifications:

  • Possession of the necessary licensure, education, training, and qualifications mandated by state regulations, along with successful completion of a competency evaluation prior to patient care.
  • Completion of a minimum of 75 hours of classroom instruction and supervised practical training in a program sanctioned by the Department of Public Health.
  • Active and valid CNA License in good standing.
  • Fulfillment of 12 hours of annual in-service education.
  • Minimum age of 18 years.
  • Ability to comprehend and execute written instructions and document care accurately.
  • Familiarity with hospice care philosophies and comfort in providing specialized support to terminally ill patients.
  • Self-motivated with the capability to work independently with minimal supervision, while being comfortable with issues surrounding death and dying.
  • Exhibits a calm demeanor when in patients' homes.
  • Empathetic towards the needs of the ill, injured, frail, and impaired individuals.
  • Demonstrates patience, tact, and maintains good personal hygiene.
  • Possesses a valid driver's license and operates an insured vehicle that meets organizational standards.

Key Responsibilities:

  • Execute personal care tasks promptly upon receiving patient assignments and communicate any concerns to the Case Manager.
  • Engage positively and ethically with patients and their families, ensuring confidentiality is upheld.
  • Prioritize patient safety and proper utilization of equipment (e.g., foot stools, side rails, oxygen).
  • Deliver personal care services including bathing, back rubs, oral hygiene, and hair washing as directed.
  • Maintain nails using an emery board; refrain from cutting or clipping them.
  • Perform light housekeeping tasks such as changing bed linens and cleaning patient areas to uphold a safe environment.
  • Assist patients with dressing, meal planning, and feeding as required.
  • Measure and document vital signs (temperature, pulse, respiration, blood pressure) with demonstrated competency.
  • Monitor and care for patients' skin to prevent breakdown over bony prominences.
  • Report changes in patient conditions to the Case Manager, including interpersonal issues involving caregivers.
  • Conduct range of motion exercises, transfers, positioning, and other simple procedures as instructed.
  • Assist with self-administered medications in accordance with state regulations and RN case manager directives.
  • Provide assistance with bedpans, urinals, or commodes, and accurately measure and record intake and output.
  • Offer respite for patients, families, or caregivers while on-site.
  • Adhere to documentation standards, care procedures, and professional conduct guidelines.
  • Participate in quality improvement initiatives.
  • Respect the environments and personal needs of patients and families.
  • Perform additional assigned duties as necessary.
