Position Overview:
The Hospice Certified Nursing Assistant (CNA) plays a vital role in supporting the interdisciplinary team under the guidance of a registered nurse. This position focuses on delivering personal care to fulfill patients' needs and enhance their comfort during a critical time in their lives. Responsibilities include adhering to care plans, monitoring and reporting patient conditions, and maintaining accurate documentation of care provided.
Essential Qualifications:
- Possession of a valid CNA license, active and in good standing, along with the necessary education and training as mandated by state regulations.
- Completion of a minimum of 75 hours of classroom instruction and supervised practical training in an approved program.
- A commitment to ongoing education, including 12 hours of annual in-service training.
- Minimum age of 18 years.
- Proficient in reading and following written instructions, as well as documenting care accurately.
- Understanding of hospice care principles and comfort in providing specialized support to terminally ill patients.
- Ability to work independently while demonstrating a calm demeanor in patients' homes.
- Empathy and sensitivity towards the needs of the ill, injured, and vulnerable.
- Strong personal hygiene and professionalism.
- A valid driver's license and a reliable, insured vehicle.
Key Responsibilities:
- Promptly perform personal care tasks as assigned and communicate any concerns to the Case Manager.
- Maintain positive and ethical communication with patients and their families, ensuring confidentiality at all times.
- Ensure patient safety and proper usage of equipment.
- Provide personal care services such as bathing, oral hygiene, and grooming as directed.
- Conduct light housekeeping duties to maintain a safe and clean environment for patients.
- Assist patients with dressing, meal planning, and feeding as needed.
- Measure and record vital signs with demonstrated competency.
- Monitor and care for patients' skin to prevent breakdown.
- Report any changes in patient condition to the Case Manager.
- Assist with mobility and positioning as required.
- Support patients with self-administered medications in accordance with regulations.
- Provide respite care for patients and their families when necessary.
- Adhere to documentation standards and professional conduct guidelines.
- Participate in quality improvement initiatives.
- Respect the personal needs and environments of patients and their families.
- Perform additional duties as assigned.
