Position Overview:
The Hospice Certified Nursing Assistant (CNA) plays a vital role in the interdisciplinary team, working under the guidance of a registered nurse to deliver essential personal care that addresses the needs of patients and enhances their comfort. Responsibilities include adhering to established care plans, monitoring and reporting on patient conditions, and maintaining accurate documentation of care provided.
Qualifications:
- Possession of the necessary licensure, education, training, and qualifications mandated by state regulations, along with successful completion of a competency evaluation prior to patient care.
- A minimum of 75 hours of classroom instruction and supervised practical training in a program sanctioned by the Department of Public Health.
- An active and valid CNA License in good standing.
- Completion of 12 hours of in-service education annually.
- Must be at least 18 years old.
- Ability to comprehend and follow written instructions and accurately document care provided.
- Familiarity with hospice care principles and comfort in delivering specialized support to terminally ill patients.
- Self-motivated with the capability to work independently with minimal supervision, and comfortable with matters related to death and dying.
- Ability to maintain a calm demeanor while in a patient's home environment.
- Empathy for the needs of those who are ill, injured, frail, or impaired.
- Demonstrates patience, tact, and maintains good personal hygiene.
- A valid driver's license and access to a reliable, insured vehicle that meets organizational standards.
Key Responsibilities:
- Promptly perform personal care tasks as assigned and communicate any concerns to the Case Manager.
- Engage positively and ethically with patients and their families, ensuring confidentiality is upheld.
- Prioritize patient safety and the proper use of care equipment.
- Deliver personal care services, including bathing, oral hygiene, and grooming as directed.
- Conduct light housekeeping tasks to maintain a safe and clean environment for patients.
- Assist patients with dressing, meal planning, and feeding as needed.
- Measure and record vital signs with demonstrated competency.
- Monitor and care for patients' skin to prevent breakdown.
- Report any changes in patient condition to the Case Manager.
- Assist with self-administered medications as per regulations and instructions.
- Provide respite support for patients and their families when present.
- Adhere to documentation standards and professional conduct guidelines.
- Participate in quality improvement initiatives.
- Respect the personal needs and environments of patients and their families.
- Carry out additional assigned duties as required.
