Social Services Advocate

Position Title: Case Manager
Department: Transitional Housing
Reports to: Social Service Supervisor
FLSA Status: Non-Exempt
Compensation: $24.49 per hour
Position Overview:
The Case Manager plays a vital role in delivering holistic case management services to individuals residing in transitional housing. This position involves managing a designated caseload and working collaboratively to align housing and employment objectives for each client while adhering to the organization's contractual commitments.
Key Responsibilities:
- Perform initial evaluations and complete required documentation upon client intake.
- Facilitate referrals for urgent services.
- Support clients with orientation and daily living tasks.
- Provide crisis intervention, counseling, and supportive services.
- Create and monitor Independent Living Plans (ILP) to track client progress.
- Assist clients in securing income and benefits.
- Connect clients with medical, mental health, and community resources as necessary.
- Participate in meetings and training sessions as mandated.
- Prepare and submit required reports.
Essential Functions:
- Ability to navigate stairs within the facility.
- Accompany clients to appointments and viewings.
Qualifications:

- Bachelor's Degree in social work or a related discipline is preferred.
- Experience with evidence-based practices and standardized assessment tools.
- Strong communication and organizational abilities.
