About the Role

Lovebird Home Care is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the highest level of care for our clients.

Key Responsibilities
  • Manage and oversee our team of care providers to ensure seamless delivery of services
  • Communicate effectively with caregivers, clients, families, and other stakeholders to ensure client satisfaction
  • Manage caregiver schedules and requests for time off to ensure adequate staffing
  • Maintain accurate client documentation and ensure compliance with all legal and company requirements
  • Participate in/conduct caregiver trainings to enhance skills and knowledge
  • Develop and implement treatment/care plans to meet client needs
  • Document and report any issues or violations to upper management
  • Make oversight visits to supervise and assess caregivers
  • Monitor client progress and make adjustments to care plans as needed
  • Collect feedback to ensure client satisfaction and implement proactive solutions
Requirements
  • Experience in home care or other health service industries preferred
  • Sales experience preferred
  • Proficiency in word processing and computer skills (Office Word, Excel, & PowerPoint)
  • Excellent communication, human relations, and organizational skills
  • Ability to travel throughout assigned communities to complete job duties
  • Driver's license
  • Two years office experience, and private duty or healthcare experience preferred
  • High school diploma required, Certified Nurse Assistant or Medical Assistant, and CPR/BLS Card
