Job Summary

The Oncology Nurse Navigator plays a vital role in coordinating patient care, ensuring timely and efficient delivery of services. This position requires a skilled and compassionate nurse to assess patient needs, identify barriers to care, and develop effective plans to overcome them.

Key Responsibilities
  • Coordinate patient care across the continuum, ensuring seamless transitions and optimal outcomes.
  • Assess patient needs, identifying physical, psychological, and social barriers to care.
  • Develop and implement patient plans, working closely with healthcare providers and staff.
  • Provide expert communication and guidance to patients and families, ensuring timely and appropriate advice.
  • Participate in committee work and data collection, contributing to the development of survivorship programs.
Requirements
  • Minimum 5 years of experience in a healthcare setting, with related experience in quality, care coordination, and oncology.
  • BSN in Nursing, preferred.
  • Current New Mexico Nursing License.
Working Conditions

This role requires close work, good vision, and dexterity to write and use a computer keyboard. Ability to move around settings of care, with occasional exposure to bloodborne pathogens.
