Oncology Nurse Navigator

The Oncology Nurse Navigator is a critical role within our Cancer Center, responsible for coordinating patient care in a timely and efficient manner. This position requires a skilled nurse with expertise in oncology, patient advocacy, and care coordination.
Key Responsibilities- Coordinate patient care across the continuum, identifying and eliminating barriers to care.
- Assess patient needs, developing and implementing individualized care plans.
- Provide expert communication and guidance to patients and families, ensuring timely and effective care.
- Collaborate with healthcare teams to ensure seamless care coordination.
- Participate in quality improvement initiatives, contributing to the development of best practices in oncology care.
- Registered Nurse (RN) with a Bachelor of Science in Nursing (BSN) degree, preferred.
- Current New Mexico Nursing License and BLS certification from the American Heart Association.
- Minimum 5 years of experience in a healthcare setting, with a focus on oncology and care coordination.
This role requires the ability to work in a fast-paced environment, with frequent interactions with patients, families, and healthcare teams. The Oncology Nurse Navigator must be able to navigate multiple settings and situations, including inpatient, specialty, and primary care environments.
