About the Role:
As a Home Health LPN at CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team of dedicated healthcare professionals is committed to providing comprehensive care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities:
- Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address any identified gaps in care.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
