Job Description: Day Service Coordinator

We are seeking a highly motivated and organized Day Service Coordinator to support our programs in Momentum WORK, Inc. This role will provide on-site support to individuals with developmental disabilities, assisting them in achieving their goals and objectives in a safe and respectful environment.

Key Responsibilities:
• Develop and implement daily schedules and plans for program participants
• Provide transportation assistance as needed
• Coordinate with vendors and community partners to ensure seamless service delivery
• Maintain accurate and up-to-date records of participant progress and outcomes

Requirements:
• High school diploma or equivalent
• At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred
• Excellent organizational and time management skills
• Ability to work effectively in a fast-paced environment
• Compassion and respect for persons with disabilities

Benefits:
• Salary: $65,000 - $85,000 per annum
