Job Title: Licensed Clinical Social Worker

We are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for developing and implementing a plan for home social work services.

The estimated salary for this position is $65,000 - $85,000 per year, based on experience and location in San Luis Obispo, CA.

About the Role:

  • Develops a counseling plan based on psychosocial assessment and short-term therapy intervention.
  • Performs psychosocial assessment with patient and/or family and provides therapeutic intervention based on findings.
  • Maintains confidentiality of privileged information.
  • Evaluates effectiveness of the Treatment Plan and modifies the plan based on changing needs of the patient.
  • Coordinates service among all relevant parties to maintain continuity of care.
  • Refers to community resources when appropriate.
  • Communicates effectively with team members, patients, families, physician, referral sources, and community agencies.
