Position Overview

The Community Support Case Manager will be instrumental in delivering holistic case management services to individuals and families within the community who are vulnerable to or have experienced violence. This position entails evaluating needs, formulating intervention strategies, coordinating resources, and advocating for clients to guarantee they receive the necessary support and assistance.

The Community Support Case Manager will collaborate closely with community partners and stakeholders to execute violence prevention and intervention initiatives. Responsibilities include conducting in-depth assessments, providing crisis intervention, and creating safety plans to address both immediate and long-term needs. Furthermore, the Community Support Case Manager will work in partnership with internal and external teams, facilitate support groups, and monitor progress towards established goals.

We seek a highly competent and empathetic professional who is committed to positively influencing the lives of individuals and families impacted by violence.

Compensation

Competitive salary ranges: Bachelor's Degree $22.00-$29.00; Master's Degree $32.21-$34.00.

Key Responsibilities

The primary duties include, but are not limited to:

  • Conducting thorough assessments to determine client needs, strengths, and challenges.
  • Designing and executing tailored intervention plans.
  • Coordinating and overseeing services to ensure clients receive adequate support.
  • Providing crisis intervention and de-escalation assistance.
  • Advocating for clients and collaborating with community partners to secure essential resources.
  • Leading support groups and educational workshops.
  • Monitoring and documenting client progress and outcomes.
  • Collaborating with internal teams and participating in meetings and training sessions as required.
  • Staying informed about best practices and emerging trends in community intervention.

Qualifications

Minimum Requirements (Knowledge, Skills, and Abilities)

  • A minimum of 5 years of experience in case management or a related field.
  • Understanding of trauma-informed care and experience with individuals affected by violence.
  • Exceptional assessment and problem-solving abilities.
  • Strong communication and interpersonal skills.
  • Ability to work effectively with diverse populations.
  • Proficiency in case management software and relevant computer applications.
  • A valid driver's license and access to reliable transportation.
  • Commitment to maintaining confidentiality and adhering to ethical standards.
  • Professional certification or licensure in social work (LCSW) is preferred.
