About the Role

We are seeking a highly motivated and compassionate Case Manager to join our Outreach Services team at PATH (People Assisting the Homeless). As a key member of our team, you will play a vital role in assisting clients in breaking the cycle of homelessness by providing individualized support and connecting them to essential resources.

Key Responsibilities
  • Conduct Intake and Needs Assessment: Meet with clients to understand their unique needs and develop a personalized plan to address their goals, including housing, income, and personal well-being.
  • Provide Referrals and Linkages: Connect clients with local resources, services, and organizations to support their progress and achievement of their goals.
  • Evaluate Progress and Adjust Plans: Regularly review clients' progress, identify areas for improvement, and adjust their plans as needed to ensure they are on track to achieving their goals.
Requirements
  • Education and Experience: A minimum of 2 years of experience in healthcare, case management, homeless services, or a related field.
  • Skills and Qualities: Excellent analytical and communication skills, ability to work effectively with diverse populations, and a strong commitment to social justice and advocacy.
