Job Summary

We are seeking a dedicated and detail-oriented Regional Health Quality Patient Experience Coordinator to join our team. In this role, you will be responsible for ensuring overall agency survey readiness and quality outcomes across our IL territory.

Main Responsibilities:

  • Perform on-site survey readiness assessments for assigned providers using designated protocols and tools.
  • Collaborate with agency leadership to develop effective actions to mitigate quality-related findings.
  • Assist agencies during state and Joint Commission surveys by facilitating continued survey readiness and assisting in the development of required action plans.
  • Maintain current knowledge of regulations affecting the agency and act as a resource to agency leadership regarding Conditions of Participation, State Minimal Standards, Joint Commission, and OSHA requirements.
  • Serve as an agency resource for quality and performance improvement activities and regulatory requirements.
  • Provide face-to-face exit conferences with Branch Manager or designee after each site visit to communicate findings and discuss plans of improvement.
  • Monitor internal quality reviews and chart audit results, providing feedback on effective action plan development to improve results.
  • Periodically perform inter-rater reliability audits on internal quality reviews and chart audits performed by agency leaders.

About You

To succeed in this role, you will need 3 years of healthcare experience, preferably in home care and/or healthcare quality related experience. You must also possess a current Driver's License, vehicle insurance, and access to a dependable vehicle. Additionally, you should have an understanding of CMS Conditions of Participation, Joint Commission Standards, and state licensing standards. Effective written and verbal communication skills, good interpersonal skills, fiscal responsibility, and accountability are also essential. Proficient computer skills with knowledge of Microsoft Word and Excel are necessary, as well as being goal-oriented and able to affect change.

The estimated salary for this position is $55,000 - $65,000 per year, depending on experience.
