Job Summary:
The Compassionate Patient Care Coordinator is a vital role within our team at Henry Ford Health System, providing exceptional patient care and ensuring seamless coordination of services. This position requires strong leadership skills, effective communication, and a commitment to delivering high-quality care.

About the Role:
This 36-hour per week position involves working midnights in our 12-hour shift schedule, with every other weekend and holiday rotations. The successful candidate will have the opportunity to work in our B2 Telemetry Unit, supporting patients and families through their healthcare journey.

Key Responsibilities:
Care Coordination: Provide leadership in coordinating patient care, ensuring that each individual receives comprehensive support throughout their treatment process.
  • Patient Education: Utilize the nursing process to provide educational resources, empower patients to take an active role in their health, and promote self-care practices.
  • Team Collaboration: Foster open communication among healthcare professionals, promoting a collaborative approach to patient care and addressing any concerns or challenges that may arise.
  • Quality Assurance: Participate in quality assurance activities, analyzing data to identify areas for improvement and implementing changes to enhance patient outcomes.

Requirements:
To be successful in this role, you should possess a strong academic background, having graduated from an accredited school of nursing, with a BSN preferred. Your clinical knowledge and analytical abilities will enable you to formulate effective nursing care plans, while your ability to communicate effectively with patients, families, and hospital personnel will facilitate smooth interactions and ensure that all parties are informed and engaged.

Compensation:
The salary for this position is estimated at $80,000 - $110,000 annually, depending on experience. In addition to a competitive salary, we offer a full-time benefit package, including a $5,000 signing bonus.
