Regional Home Health Quality Patient Experience Coordinator

Job Summary
The Regional Home Health Quality & Patient Experience Coordinator is responsible for facilitating overall agency survey readiness and quality outcomes. This role oversees our IL territory and is accountable for ensuring compliance with regulatory requirements.
Key Responsibilities
• Perform on-site survey readiness assessments for each assigned provider using designated protocols and tools.
• Assist the agency leadership in developing effective actions to mitigate quality-related findings.
• Assist the agencies during state and Joint Commission surveys by facilitating continued survey readiness and assisting in the development of the required action plan.
• Maintain current knowledge of regulations affecting the agency and act as a resource to agency leadership regarding Conditions of Participation, State Minimal Standards, Joint Commission, and OSHA requirements.
• Serve as an agency resource regarding quality and performance improvement activities and regulatory requirements.
• Provide face-to-face exit conferences with Branch Manager or designee after each site visit to communicate findings and discuss plans of improvement.
• Monitor internal quality reviews and chart audit results and provide feedback on effective action plan development to improve results.
• Periodically perform inter-rater reliability audits on internal quality reviews and chart audits performed by agency leaders.
• Review and monitor Home Health Compare outcome and process measures and assist agency leadership in prioritizing performance improvement activities.
• Submit provider summary report of on-site activities and recommendations after each visit.
• Submit monthly report to PI Manager.
• Review quarterly Quality and Performance Improvement Executive Summary Reports, which includes an analysis of data and specific actions taken to mitigate, and provide appropriate guidance to agency leaders to ensure adequacy.
• Regularly communicate progress and concerns with the respective State Director and PI Manager.
• Adequately prepare for and actively participate in quality review calls with Manager.
• Perform other assigned duties timely and adequately.
Requirements
• 3 years healthcare experience, preferably home care and/or healthcare quality related experience.
• Current Driver's License and vehicle insurance, and access to a dependable vehicle.
• Understanding of CMS Conditions of Participation, Joint Commission Standards and state licensing standards.
• Effective written and verbal communication skills.
• Good interpersonal skills.
• Fiscal responsibility and accountability.
• Proficient computer skills with knowledge of Microsoft Word and Excel.
• Goal-oriented and ability to affect change.
