About the Job:
We are seeking a skilled Nurse Patient Transfer Specialist to join our team at Henry Ford Health System. This role will be responsible for coordinating patient transfers in a safe, successful, and efficient manner.
The ideal candidate will have a strong background in nursing and excellent communication skills. They will also be able to work effectively in a fast-paced environment and make sound decisions under pressure.
Responsibilities:
- Facilitate patient transfers from one healthcare facility to another
- Coordinate with hospital staff, physicians, and other healthcare professionals to ensure seamless transitions
- Evaluate patient needs and develop individualized care plans
- Maintain accurate records and reports
Requirements:
- Bachelor of Science in Nursing degree required
- Two years of RN clinical experience preferred
- Ability to work in a high-stress environment and make sound decisions under pressure
- Excellent communication and interpersonal skills
