The Comprehensive Trauma Clinic at UCLA Health seeks a skilled Community Health Worker to join our team. As a key member of the Trauma Team, this individual will play a vital role in ensuring patients receive essential assistance as they transition from trauma patients back into the community.

The primary responsibility of this position is to reduce the chance of repeat trauma, address social determinants that contribute to traumatic injury, and improve the quality of life for post-trauma patients. To achieve this, the Community Health Worker will engage in activities that promote, maintain, and improve the health and wellbeing of our patients.

Key responsibilities include:

  • Establishing relationships with admitted trauma patients who are identified as likely to need further care through the Clinic once discharged.
  • Initiating and establishing connections to community-based organizations in the Los Angeles area to provide services for our trauma patients.
  • Providing close shepherding through these resources to ensure a seamless continuity of care.

Examples of services that may be provided include mental health services, job readiness training or counseling, navigation of victim of crime compensation, GED preparation, legal advocacy, housing agencies, and addiction treatment.

Qualifications for this position include a Bachelor's degree in social work from a program approved by the Council on Social Work Education or an equivalent combination of education and experience. BLS certification is also required.
