Trauma Surgery Community Health Worker Specialist

Job Summary:

The Surgical Clinic Community Health Worker plays a vital role as part of the Trauma Team and Comprehensive Trauma Clinic program at UCLA Health. This position ensures patients receive essential assistance as they transition from trauma patients back into the community, reducing their chance of repeat trauma, addressing social determinants that contribute to traumatic injury and other poor health outcomes, and improving their quality of life.

Key Responsibilities:
  • Engage in activities that promote, maintain, and improve the health and wellbeing of post-trauma patients.
  • Work primarily in the outpatient setting through the Comprehensive Trauma Clinic, establishing relationships with admitted trauma patients who are identified as likely to need further care through the Clinic once discharged.
  • Initiate and establish connections to community-based organizations in the Los Angeles area to provide services for trauma patients and provide close shepherding through these resources.

Requirements:
  • Bachelor's degree in social work from a program approved by Council on Social Work Education or equivalent combination of education and experience.
  • BLS Required.
