Job Title: Community Care Advocate

As a Community Care Advocate, you'll play a critical role in connecting employees to community and company resources that meet their social determinants of health and needs.

Key Responsibilities:
  • Identify and refer employees to work-life, financial, and legal support resources
  • Link employees to local and national resources through one-on-one consultations and referral lists
  • Work with internal care teams to provide emergency funding and resource referrals
  • Offer consultation on navigating mental health resources and workplace benefits
  • Assist employees in linking to mental health treatment options and provide support throughout the process
  • Document and track metrics on referrals, demographics, and outcomes
Requirements:
  • Masters in Social Work from an accredited university
  • Experience working with diverse populations and backgrounds
  • Ability to balance multiple stakeholder needs
  • Flexibility and creativity in approaching workplace challenges
  • Excellent communication and listening skills
