Job Summary

Lighthouse Behavioral Health Solutions (LBHS) is seeking a qualified Care Coordinator to provide case management services to adults with identified mental health or substance use diagnosis.

Key Responsibilities
  • Provide ongoing case management services to residents, including regular check-ins, goal setting, and progress monitoring.
  • Serve as a liaison between the recovery housing program and treatment providers, facilitating communication and information sharing regarding case management services.
  • Coordinate and schedule appointments with treatment providers, ensuring residents have access to necessary services and follow-up care.
  • Assist residents in accessing community resources and support services, such as employment assistance, educational programs, and legal services.
  • Advocate for residents' needs and rights, ensuring they receive appropriate and quality care.
Requirements
  • CDCA or LCDCII certification
  • State of Ohio Driver's License
  • Valid BCI check, all Corporate Compliance checks, and employment drug screen
  • Must be 25 years or older
Work Environment

Tuesday - Saturday 7am - 3pm (hours and days may vary depending on business need)
