Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our Case Management Team. As a trusted member of the community, you will work to improve the health of individuals with behavioral health needs by providing culturally and linguistically appropriate services.

Key Responsibilities
  • Establish trusting relationships with clients and their significant others while providing general support and encouragement.
  • Provide ongoing support and engagement, using motivational interviewing and person-directed goal setting.
  • Assist individuals in identifying and navigating to and through various health and social resources that will help clients achieve better health outcomes.
  • Collaborate with clients, clinical staff, and community partners to empower clients in securing the services and support they need.
  • Follow-up with clients via phone calls, home visits, and visits to other settings frequented by those needing care.
  • Help individuals connect with transportation resources and provide appointment reminders as needed.
  • Attend client appointments and advocate for client needs as needed.
  • Work collaboratively with medical providers and clinical staff to ensure clients have comprehensive and coordinated care plans.
  • Assist clients and/or teach the skills necessary for clients to access community resources.
  • Provide timely and accurate documentation.
  • Coordinate care with medical providers, housing and residential programs, sobering centers, substance abuse treatment agencies, CRC, senior and disabled services, and other service providers.
Requirements
  • Must be currently certified, or eligible to become certified within 6 months of hire, by the State as a Community Health Worker and listed in the Traditional Health Worker (THW) registry.
  • Education and experience providing behavioral health and/or social supports to vulnerable populations and knowledge of local resources preferred.
  • Demonstrated ability to interact positively and in a culturally appropriate manner with a wide range of communities.
  • Member of and/or unusually close understanding of the communities served.
  • Candidates must be able to use email, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system.
  • Must pass state-required background and DMV checks; possess a valid OR driver's license and a satisfactory driving record to transport clients.
