Community Health Worker

At Mercy Flights Inc, we are seeking a highly motivated and compassionate Community Healthcare Worker to join our team. As a Community Healthcare Worker, you will play a vital role in providing short-term care coordination and connection to resources and support to program clients to improve their health and general well-being.
Key Responsibilities:- Work closely with the MIH team, primary care teams, and social services agencies to provide quality patient care and coordinate services.
- Build and maintain positive working relationships with clients, providers, nurse case managers, agency representatives, supervisors, and office staff from diverse cultural and socio-economic backgrounds.
- Provide education and coordination of care and services to program clients to improve their health and general well-being.
- Maintain comprehensive electronic client files, which include client notes, the release of information, assessments, and other medical documents acquired on behalf of the client.
- Complete patient care reports promptly per Oregon State law company protocols and guidelines.
- A High School Diploma or equivalent is required.
- Oregon Driver's License
- CPR certification
- An associate degree in the health care or human services is preferred. A combination of education and experience will be considered.
- Any combination of one (1) year of health care/social services experience and/or education is required; two (2) years is preferred.
- Experience in the substance abuse and/or mental health field is preferred.
- Community Health Worker certificate (required to obtain within one year of start date).
- Full-time schedule with a rotating schedule that promotes work-life balance and weekends off.
- Daily overtime if shift exceeds 12 hours.
- Starting Pay rate: Market comparable (Depending on Qualifications).
