About the Role

We are seeking a skilled Registered Nurse to join our Home Health team at Providence. As a Home Health Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Assess patients' physical, emotional, and social needs, and develop individualized care plans to meet those needs.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct home visits to assess patients' needs and provide care, as needed.
  • Document patient progress and communicate with healthcare teams to ensure continuity of care.
Requirements
  • Current Oregon Registered Nurse License.
  • 1 year of medical/surgical experience in an acute care setting required for Home Health RN.
  • 2 years of pediatric experience in an acute care setting for RN caring for pediatric patients.
  • National Provider BLS - American Heart Association certification.
