Join Our Team as a Social Worker - Chronic Care Team

We are seeking a skilled and compassionate Social Worker to join our Chronic Care Team at the Lynn Community Health Center. As a Social Worker, you will play a vital role in addressing the social service needs of our health center clients referred by the Primary Care/BH Team.

Key Responsibilities:
  • Provide follow-up sessions as needed with health center clients or coordinating services by phone.
  • Provide brief counseling, advocacy, & resource referrals as needed.
  • Participate in case conference as needed.
  • Conduct home visits alone or in conjunction with member(s) of the interdisciplinary team as appropriate, either at the patient's home or in other locations in the community.
  • Be an active & integral member of the primary care/BH team.
  • Advocate for patients with outside agencies such as social service organizations, schools, etc.
  • Provide crisis intervention as needed.
Requirements:
  • Human services experience desired.
  • Ability to work with children & adults, & the elderly.
  • Minimum of a bachelor's degree in the human service field (e.g., psychology, sociology) required, BSW preferred; bilingual/bicultural candidates strongly desired.
  • LSW or License eligible - the Social Worker is expected to work progressively towards attaining LSW status
  • History of good attendance & positive work attitude.
