Position Overview

As the Community Services Team Leader, you will oversee, mentor, and guide our team of Community Leads, remote care coordinators, and part-time outreach personnel. You will play a vital role within the Operations Leadership Team at Ounce of Care.

Your team of Community Leads acts as the primary representatives of Ounce within our partner housing communities, engaging directly with residents on a daily basis. They are trained across various functions, including outreach, enrollment, clinical care coordination, and assistance with insurance and benefits navigation. You will lead these teams, engage with our partners, and collaborate across the broader Ounce of Care organization to enhance service delivery and support within affordable housing.

Ideal Candidate Profile:
  • Experienced Leader: You have successfully built and led teams in dynamic environments, ideally in community-based service delivery or clinical/social care settings.
  • Healthcare Background: You possess experience in roles such as care management, case management, or community health work, and are familiar with the relevant workflows.
  • Hands-On Approach: You are enthusiastic about observing, coaching, and managing a dispersed team, and you take pleasure in seeing your team interact with residents while providing real-time support.
  • Value-Driven: You are motivated by solving challenges that align with your personal values.
  • Flexible: You thrive in environments with minimal supervision and can adapt to new information and challenges effectively.
Additional Information:
  • Employment Type: Full-time
  • Commute: Ability to travel locally as needed to manage and support teams.
  • Travel: Occasional travel to other markets may be required for oversight, training, or management.
  • Working Hours: Primarily during EST, with flexibility for CST and PST as necessary.
