Position Overview:
The RN Admissions Nurse plays a crucial role in the admissions process, reporting directly to the Director of Admissions. This position is responsible for conducting patient evaluations and facilitating admissions to Care Dimensions, ensuring a seamless transition to ongoing care provided by our interdisciplinary team. Additionally, the RN Admissions Nurse educates healthcare professionals, patients, and families about the benefits and appropriateness of hospice care across various settings.

Key Responsibilities:

  • Conduct thorough patient assessments and manage the admissions process.
  • Maintain effective communication with clinical staff to enhance hospice services.
  • Facilitate communication between Care Dimensions and referral sources.
  • Provide education to healthcare professionals, patients, and families regarding hospice care.

Qualifications:
  • Current Registered Nurse license in Massachusetts, in good standing.
  • Associate's degree in nursing required; Bachelor's degree preferred.
  • Professional nursing experience, ideally with at least one year in a skilled or acute care setting.
  • Proven ability to assess and address the needs of patients and families in diverse environments.
  • Self-directed, adaptable, and cooperative with a strong ability to work within an interdisciplinary team.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.

Compensation and Benefits:
The anticipated salary range for this position is between $80,000 and $110,000 USD, determined by factors such as education, experience, and location.
